- Screening responses (age, symptoms, forward bend test observations)
- Contact information (name, email, phone) if you choose to provide it
- Photos if you choose to upload them for screening review
How We Use Your Information
Your screening information is used to generate your educational risk report and, if you provide contact information, to facilitate follow-up from our scoliosis specialists. Screening results are sent to Clear Life Scoliosis & Chiropractic Center via email for review.
Data Storage & Security
Screening data is processed in real-time and sent via email. We do not maintain a persistent database of screening results. Uploaded photos are stored securely and can be deleted upon request. We use industry-standard security measures to protect your information during transmission.
Your Rights
You may request deletion of any uploaded photos or contact information by emailing [email protected]. You are not required to provide any personal information to use the screening tool.
